Family Support Hub Developers have agreed a standard referral form for accepting referrals for children, young people, parents or families to a Family Support Hubs in Northern Ireland. This form will ensure the same data is being collected in the same way across all hubs in relation to the referred party in compliance with Health and Social Care Board Ethnic Monitoring standards.

This referral form will also provide a baseline to identify children’s needs and gaps in current service provision in Northern Ireland which will enable Locality Planning Groups within Hub areas to plan to address these areas of need. Attached is a template of what the referral form will look like and the information that will be required when referring to a hub.

Family Support Hub members from the Southern Area who met at a workshop in Armagh (November 2012), to share ideas on what is working and what is not and offer recommendations to improve referral processes to Family Support Hubs.
